Description Brief: Port Authority Reports or documentation relating to irregularities.

This was originally sent to the Port Authority and then also forwarded onto our Office.

The full request was as follows:

Good day Dear FOI Manager,

I understand that a letter, or email was recently issued to Port Authority's staff advising of some "concerning irregularities in Human Resources, Finance and other areas."

I am hereby making an FOI request for the following:

1) an electronic copy of the letter, or email that was sent to the Port Authority's staff relative to the concerning irregularities.

2) What is the specific nature of the concerning irregularities?

3) Is an investigation being carried out, or has been carried out and by who?

4) Are there any reports or documentation relating to these irregularities?

5) Are those irregularities a breach of any particular law or laws and if so, which laws and is the police involved?

6) If there have been no breach of laws, is there any documents or reports available outlining the nature of the irregularities that I can request an electronic copy of?

7) Has any staff member, or members been identified as being responsible for those irregularities, or what action is being taken to identify those responsible for the irregularities?

8) Has this matter been considered by the Board of the Port Authority?

9) What specific action is the Board and Management of the Port Authority taking to address the irregularities concerned?

10) Can I request electronic copies of all Board Minutes relative to this matter, unless it is a police matter?

11) Has anyone been suspended, dismissed, or subject to police action for these "concerning irregularities?"
